Key Responsibilities:

  • Manage and coordinate care for assigned patients in an outpatient clinic setting, ensuring individualized medical goals are met to optimize healthcare outcomes.
  • Collaborate with medical providers and interdisciplinary team members to develop and implement care plans.
  • Facilitate multi-disciplinary care conferences for high-risk or complex patients referred into case management.
  • Monitor patient panels for acute and chronic conditions, providing outreach and education based on electronic health record reminders and triggers.
  • Participate in case reviews with a multidisciplinary team, focusing on high-need patients.
  • Advocate for patients and families, promoting decision-making, independence, and self-care.
  • Coordinate patient referrals within the organization and external facilities, tracking progress and outcomes.
  • Serve as primary liaison for other case managers and discharge coordinators to ensure seamless care transition.
  • Perform direct nursing duties including assessment, documentation, patient education, and medication administration in a fast-paced outpatient environment.
  • Maintain safety and infection control standards, ensure a clean and safe working environment, and report safety hazards.
  • Support department improvement efforts and participate as an active team member.
  • Respond to emergency situations and participate in triage as needed.

Required Qualifications:

  • Active, unrestricted Registered Nurse license in Alaska.
  • Graduated from an accredited nursing program.
  • Basic Life Support (BLS) certification.
  • At least 1 year of recent case management experience (within the last 2 years).
  • Knowledge of healthcare services, patient advocacy, safety, and infection control principles.
  • Proficiency in medical and computer equipment, assessment, and data collection.
  • Strong communication skills, ability to multi-task, and work independently in a fast-paced environment.

Preferred Experience / Skills:

  • Bachelor’s degree in Nursing (preferred).
  • National Case Management certification.
  • Familiarity with tribal health services in Alaska.
  • Experience with eHealth systems and care coordination within a healthcare setting.
