Key Responsibilities

• Perform patient assessments and treatments in a long-term care facility.
• Administer medications and notify physicians or nurse practitioners of pertinent patient information.
• Assist with patient admissions and discharges.
• Provide assistance with Activities of Daily Living (ADLs).
• Deliver specialized care including gastrostomy tube, urinary catheter, wound vac, CPAP, tracheostomy, and nebulized treatments.
• Maintain documentation and ensure patient safety during all procedures.
• Work scheduled 8-hour shifts, including weekends and holidays, with flexibility required for varying hours.

Required Qualifications

• Valid LVN or LPN license.
• Current CPR certification.
• Experience in long-term care or skilled nursing facilities preferred.
• Ability to perform complex patient care procedures.
• Strong communication skills and patient care focus.

Preferred Experience / Skills

• Prior experience with wound care, tube management, and ventilator support.
• Knowledge of infection control standards.
• Flexibility to work varied shifts, including weekends and holidays.
