Key Responsibilities

• Provide personalized rehabilitative services and evaluations to outpatient residents and the local community.
• Track and document patient progress using facility systems.
• Foster long-term relationships with residents to support their well-being.
• Guide patients, families, and staff through therapy programs, goals, and outcomes.
• Maintain a professional, compassionate, and ethical demeanor.
• Collaborate with team members to ensure high-quality patient care and positive outcomes.

Required Qualifications

• Degree in Physical Therapy from an accredited program.
• Active Pennsylvania PT license in good standing.
• Passion for working with the senior population.
• Strong communication and collaboration skills.
• Positive, team-oriented approach to patient care.

Preferred Experience / Skills

• Experience working with older adults or in a senior living or healthcare setting.
• Familiarity with outpatient therapy documentation systems.
• Ability to build rapport with residents and their families.
