Key Responsibilities
• Conduct assessments, develop, implement, and update individualized care plans in collaboration with patients, families, physicians, and interdisciplinary teams.
• Provide direct nursing care, manage pain and symptom control, and ensure medication and supply needs are met cost-effectively.
• Document patient status, care activities, and communications promptly and accurately using appropriate documentation practices.
• Participate in interdisciplinary team meetings, sharing information and collaborating on care strategies to meet patient needs.
• Supervise Home Health Aides and Licensed Practical Nurses in patient care activities.
• Facilitate patient and family education, referrals to community agencies, and support timely admissions and service deliveries.

Required Qualifications
• Active Registered Nurse license in Wisconsin and/or Michigan (additional license required within 90 days if applicable).
• Current Basic Life Support (BLS) certification.
• Valid driver’s license with automobile liability insurance of at least $100,000/$300,000 coverage.
• Ability to travel to patient homes as needed.
• Knowledge of nursing standards of care and comprehensive assessment skills.

Preferred Experience / Skills
• Previous home health or hospice experience preferred.
• Broad-based nursing experience of at least five years.
• Additional training or experience working with terminally ill patients advantageous.
