Key Responsibilities
• Conduct evaluations, plan and implement individualized treatment programs for adult-geriatric patients.
• Manage caseloads involving post-operative orthopedic cases, falls, CVA, and weakness.
• Document patient progress and treatment plans accurately in electronic medical records.
• Collaborate with a multidisciplinary team to ensure quality patient care.
• Assist with discharge planning and patient education.
• Adapt therapy hours with a flexible schedule between 7 AM and 6 PM, Monday through Saturday.

Required Qualifications
• Valid state license as a Physical Therapist.
• Experience with adult-geriatric populations, including post-op orthopedic and neuro cases.
• Proficient in documentation and electronic medical records.
• Strong communication and team collaboration skills.

Preferred Experience / Skills
• Experience in a skilled nursing facility or rehab setting.
• Knowledge of Net Health EMR or similar systems.
• Bilingual abilities are a plus.
