• Part-time, Days, 5 days per week, approximately 32 hours guaranteed

Key Responsibilities

• Provide skilled physical therapy services to residents in an assisted living environment
• Assess residents' needs and develop personalized treatment plans
• Implement therapy programs to improve mobility, strength, and overall function
• Collaborate with healthcare team to monitor progress and adjust treatment as needed
• Document patient progress and maintain accurate records in compliance with regulations

Required Qualifications

• Valid state-issued Physical Therapist license or certification
• Proven experience in providing outpatient or assisted living therapy services
• Strong understanding of therapeutic techniques and patient assessment
• Excellent communication skills and ability to collaborate with multidisciplinary teams

Preferred Experience / Skills

• Experience working in assisted living or long-term care settings
• Knowledge of age-related health conditions and mobility issues
• Ability to adapt therapy plans to individual resident needs
