Key Responsibilities
• Develop and implement care plans in collaboration with physicians, patients, and families.
• Assess, monitor, and document patients' clinical, functional, and emotional status regularly.
• Coordinate care activities through interdisciplinary communication and case conferences.
• Educate patients and caregivers on self-care techniques, medication, diet, and health management.
• Communicate significant changes in patient conditions to physicians and team members promptly.
• Provide ongoing health counseling and ensure treatment goals are met.
Required Qualifications
• Current RN license in Vermont or New Hampshire, or an unencumbered compact license.
• Graduation from an accredited nursing program.
• Minimum of 2 years of experience in home health nursing.
Preferred Experience / Skills
• Experience working in hospice or interdisciplinary home health settings.
• Strong communication and patient education skills.
