Key Responsibilities

  • Manage active patient care in collaboration with medical providers and interdisciplinary teams to achieve optimal health outcomes.
  • Coordinate multidisciplinary care conferences for high-risk or complex patients and ensure timely patient follow-ups.
  • Assist in developing individualized care plans, review electronic health records, and provide patient outreach and education.
  • Serve as a patient advocate, promoting self-care, independence, and decision-making at the service delivery level.
  • Facilitate patient referrals within the organization and external facilities, tracking progress and outcomes of care.
  • Perform clinical nursing duties, including assessments, documentation, education, and administering therapies or medications.
  • Support specialty clinics by managing referrals, scheduling, and assisting during medical procedures.
  • Act as the primary nurse for obstetrics-related case management, including patient education, postpartum tracking, and coordination of newborn immunizations.
  • Coordinate telehealth visits and diagnostic tests, ensuring smooth communication between providers and patients.

Required Qualifications

  • Active, unrestricted Registered Nurse license in Alaska at the time of application.
  • Valid Alaska-specific licensure and certification that meets state nurse practice requirements.
  • Minimum of 1 year of case management experience within the last 2 years.
  • Proof of current COVID-19 vaccination (including flu vaccine if required).
  • Basic Life Support (BLS) certification.

Preferred Experience / Skills

  • Certification in case management (e.g., CCM).
  • Knowledge of tribal health services and community-specific healthcare needs in Alaska.
  • Strong assessment, problem-solving, and multi-tasking skills in a fast-paced environment.
  • Excellent communication skills, both oral and written, with a customer service focus.
  • Ability to operate medical equipment and electronic health record systems efficiently.
