Key Responsibilities
- Provide skilled nursing care to residents in a Long Term Acute Care (LTAC) setting.
- Communicate promptly with physicians, families, and team members about significant changes in patient condition.
- Complete all applicable resident assessments accurately and timely.
- Administer medications following the 5 Rights to ensure safety.
- Perform wound care and assessments as needed.
- Assist CNA staff with answering call lights and resident needs.
- Monitor residents for changes in condition and respond accordingly.
- Lead on the floor by assessing situations and directing appropriate actions.
- Report any allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator.
- Maintain detailed and accurate resident records through thorough charting.
- Perform additional duties as required to support resident care.
Required Qualifications
- Two-year nursing degree (Associate’s Degree in Nursing required; Bachelor’s preferred).
- Valid and current Wisconsin Registered Nursing license.
- Current CPR/BLS certification.
- At least 1 year of related work experience preferred.
- Exceptional attention to detail and accuracy in documentation and care.
