Key Responsibilities

• Provide skilled nursing care to residents in a Long Term Acute Care (LTAC) setting.
• Communicate promptly with physicians, families, and team members about significant changes in patient condition.
• Complete all applicable resident assessments accurately and timely.
• Administer medications following the 5 Rights to ensure safety.
• Perform wound care and assessments as needed.
• Assist CNA staff with answering call lights and resident needs.
• Monitor residents for changes in condition and respond accordingly.
• Lead on the floor by assessing situations and directing appropriate actions.
• Report any allegations of abuse, neglect, or misappropriation to the Director of Nursing and Administrator.
• Maintain detailed and accurate resident records through thorough charting.
• Perform additional duties as required to support resident care.

Required Qualifications

• Two-year nursing degree (Associate’s Degree in Nursing required; Bachelor’s preferred).
• Valid and current Wisconsin Registered Nursing license.
• Current CPR/BLS certification.
• At least 1 year of related work experience preferred.
• Exceptional attention to detail and accuracy in documentation and care.
