Key Responsibilities
- Provide compassionate hospice nursing care to patients in their homes, skilled nursing facilities, assisted living, and adult foster homes within the service areas.
- Administer medications, manage symptoms, and monitor patient health to ensure comfort and quality of life.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Offer emotional and spiritual support to patients and their families during end-of-life stages.
- Maintain thorough, accurate documentation of patient care activities and interventions.
- Ensure compliance with all healthcare regulations and organizational policies.
Required Qualifications
- Current unencumbered Oregon RN license.
- Healthcare Provider Level BLS certification.
- Valid driver’s license and auto insurance, with reliable personal transportation for visits.
- Ability to work independently and provide empathetic patient care.
Preferred Experience / Skills
- BSN degree or higher preferred.
- Previous hospice, home health, or case management experience.
- Broad-based nursing skills with experience in palliative or outpatient care.
