Job Title
• LVN / LPN - Long-Term Care / Skilled Nursing Facility

Schedule / Job Type
• Night shift, full-time, variable shifts (8-hour or 12-hour shifts)

Key Responsibilities
• Provide skilled nursing care to geriatric patients in a Long-Term Care / Skilled Nursing Facility setting.
• Follow patient care plans and maintain accurate documentation using electronic medical records.
• Support patient daily needs and monitor health status during night hours.
• Collaborate with healthcare team to ensure quality patient care and safety.
• Participate in building orientation sessions prior to shift, including shadowing when possible.
• Adhere to facility policies, including weekend rotations and holiday staffing as needed.

Required Qualifications
• Valid LVN / LPN license in the state of Nebraska.
• Experience working in long-term care or skilled nursing facilities preferred.
• Ability to work night shifts and adapt to variable schedules.
• Proficiency in electronic medical records, preferably Point Click Care.
• Strong patient care skills and attention to detail.

Preferred Experience / Skills
• Prior experience with geriatric patient populations.
• Knowledge of LTC facility operations and common diagnosis types.
