Key Responsibilities

  • Provide hospice and end-of-life care to adult patients, with occasional pediatric cases, in home and inpatient settings.
  • Manage complex symptom assessments and interventions, including pain management and medication administration via various routes (IV, subcutaneous, oral, rectal, etc.).
  • Participate in goals of care discussions with patients, families, and multidisciplinary teams to ensure comprehensive end-of-life planning.
  • Document patient care using a narrative-heavy EMR system, maintaining high standards of accuracy and completeness.
  • Cover call responsibilities as part of the team, with infrequent on-call duties.
  • Support patients and families through education on symptom management and comfort measures.

Required Qualifications

  • Current BLS certification (American Heart Association required)
  • Minimum of 3 years of recent community hospice experience
  • Strong clinical assessment skills with the ability to handle complex symptom management
  • Proficiency with EMR documentation systems (experience with Net Smart preferred)
  • Valid driver’s license with reliable transportation (personal or rental vehicle recommended; all-season or snow tires during winter months)
  • Ability to work scheduled shifts including evenings, weekends, and holidays as assigned

Preferred Experience / Skills

  • Experience managing ventilators or advanced pain management devices (e.g., CADD pumps, feeding pumps, BiPAP/CPAP)
  • Strong critical thinking skills with the ability to handle high-acuity cases
  • Excellent communication skills, including participation in family and team care planning
  • Prior experience working in inpatient hospice units and in varied community settings
