Job Title

Key Responsibilities
• Provide outpatient physical therapy services to patients in a critical access hospital setting
• Develop and implement individualized treatment plans to improve patient outcomes
• Document patient progress and treatment details using Cerner charting system
• Collaborate with healthcare team members to ensure comprehensive patient care
• Adapt schedules based on patient needs and facility requirements

Required Qualifications
• Minimum of 1 year of experience as a licensed physical therapist
• BLS certification required
• Open to first-time therapists and new graduates
• Ability to discuss and accommodate scheduling requests with management
• Valid state licensure or eligibility for licensure in Illinois

Preferred Experience / Skills
• Experience working in outpatient rehabilitation or similar setting
• Familiarity with Critical Access Hospital environment
• Strong communication and team collaboration skills
