Registered Nurse - Home Health
Location: Le Mars, IA
Schedule / Job Type: Full-time, Days, Onsite
Key Responsibilities:
- Provide patient care in clients' homes within Plymouth County, Iowa.
- Perform OASIS assessments and develop individualized care plans for in-home patients.
- Participate in departmental call rotations, approximately one out of four shifts.
- Document patient interactions and care plans accurately using electronic medical records.
- Collaborate with healthcare team members to ensure quality patient outcomes.
- Support patients' health education and promote safety in the home environment.
Required Qualifications:
- Valid registered nurse (RN) license issued by the state of Iowa.
- Minimum of 2 years experience performing OASIS assessments and care planning.
- Familiarity with electronic medical records, preferably Meditech.
- Ability to work independently and manage a diverse caseload.
- Excellent communication and patient care skills.
Preferred Experience / Skills:
- Previous home health or hospice experience.
- Experience with Meditech EMR is a plus.
- Strong organizational skills and attention to detail.
