Registered Nurse - Hospice
Location: Moab, UT
Schedule / Job Type: Full-time, Days shift, Contract (with on-call responsibilities)

Key Responsibilities:

• Perform comprehensive assessments of patients’ needs and coordinate individualized care plans.
• Provide pain and symptom management in accordance with the hospice plan of care.
• Educate and support patients and families regarding disease process, self-care techniques, end-of-life issues, and hospice philosophy.
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and volunteers.
• Participate in a rotating on-call schedule, responding within 45 minutes to patient needs, including some weekends and holidays.
• Maintain accurate, timely documentation compliant with state, federal, and organizational policies.

Required Qualifications:

• Valid Registered Nurse (RN) license in Utah or a compact state license.
• Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care.
• Strong clinical assessment skills.
• Excellent communication and organizational skills.
• Compassionate demeanor, with the ability to support patients and families during difficult times.
• Valid driver’s license and reliable transportation for home visits; mileage reimbursement applicable.
• Current BLS certification.

Preferred Experience / Skills:

• Experience with Hospicesoft or Careficient EMR systems is preferred.
• Experience working in a fast-paced, community-based hospice setting.
