Key Responsibilities

• Provide nursing care to patients in skilled nursing units, collaborating with interdisciplinary team members.
• Administer prescribed medications and treatments in accordance with policies, procedures, standards of care, and state law.
• Document patient care accurately and timely, following standards of documentation.
• Develop, initiate, and execute individualized care plans based on patient needs, including patient and family education.
• Participate in department performance improvement activities and support shared governance initiatives.
• Use clinical expertise and guidance to support a patient's comprehensive care plan across departments.
• Practice critical thinking to ensure patient safety and quality care, escalating concerns appropriately.

Required Qualifications

• Valid Wisconsin Registered Nurse license.
• Current CPR certification.
• Experience in a skilled nursing or long-term care setting preferred.

Preferred Experience / Skills

• Previous experience working in a LTC or SNF environment.
• Strong teamwork and communication skills.
• Ability to adapt to a rotating schedule between weekday and weekend shifts.
