Key Responsibilities

• Provide outpatient physical therapy services to patients in a split between Holden Manor and Meadow View facilities.
• Develop and implement individualized treatment plans to improve patient mobility and function.
• Monitor and document patient progress, adjusting treatments as necessary.
• Collaborate with healthcare team members to ensure optimal patient care.
• Educate patients and families on treatment goals and home exercise programs.

Required Qualifications

• Valid and current license as a Physical Therapist.
• Proven experience in outpatient therapy settings.
• Strong understanding of therapeutic techniques and patient care protocols.
• Excellent communication and interpersonal skills.
• Ability to work effectively within a team environment.

Preferred Experience / Skills

• Previous experience working in skilled nursing or senior care facilities.
• Knowledge of electronic health record (EHR) systems.
• Ability to handle a split shift and manage a diverse patient caseload.
