Key Responsibilities
• Collaborate with an interdisciplinary team to develop and implement personalized care plans.
• Provide one-on-one occupational therapy services to patients in their homes.
• Educate and support clients in setting and achieving their functional goals.
• Assist clients in maintaining independence and living safely in their own homes.
Required Qualifications
• Current Vermont occupational therapy licensure.
• Experience in home health care preferred.
• Familiarity with EMR systems such as McKesson/Netsmart is a plus.
• Availability to commit without requested time off.
Preferred Experience / Skills
• Prior experience delivering therapy services in a home health setting.
• Strong communication and client engagement skills.
