Key Responsibilities

• Provide nursing care to residents in a 34-bed LTC/SNF unit within an acute care setting
• Float between the LTC unit and acute hospital as needed without call requirements
• Ensure all documentation and medication procedures are completed accurately using Sigmacare and Epic systems
• Maintain patient safety and comfort, adhering to unit protocols and procedures
• Collaborate with interdisciplinary team to meet patient care needs and schedules
• Support patient care in varied diagnoses and age ranges from 45 to 99+

Required Qualifications

• Active NY LVN/LPN license (or proof of application with applied date)
• Minimum of 1 year of clinical experience in long-term care or SNF setting
• Strong documentation skills and familiarity with Sigmacare and Epic systems
• Ability to work independently and manage multiple responsibilities
• Reliable transportation (personal or rental vehicle) with all-season tires, especially during winter months

Preferred Experience / Skills

• Prior travel nursing experience preferred but not required
• Experience with residents of varied ages and complex care needs
• Knowledge of IV administration and basic invasive monitoring tools (if applicable)
