Key Responsibilities
- Provide skilled home health nursing care to patients in their residence.
- Assess patient needs and develop individualized care plans in collaboration with the healthcare team.
- Administer treatments and medications following physician orders and agency policies.
- Maintain accurate and timely documentation using EPIC and Oasis systems.
- Educate patients and their families on health management and disease prevention.
- Ensure compliance with all applicable regulations and safety protocols.
Required Qualifications
- Active and pending state nursing license accepted.
- Minimum of 1 year of skilled home health nursing experience.
- BCLS certification.
- Own transportation for covering assigned patient visits.
- Experience with Oasis documentation system.
Preferred Experience / Skills
- Prior experience in a home health setting.
- Familiarity with EPIC and Oasis documentation systems.
