Key Responsibilities
- Provide skilled nursing care to patients in their homes, ensuring adherence to care plans.
- Conduct patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed.
- Document patient progress and treatment details using EPIC/Oasis systems.
- Collaborate with healthcare team members to coordinate patient care.
- Maintain accurate and timely record-keeping in compliance with facility standards.
Required Qualifications
- Minimum 1 year of skilled home health experience.
- Current state RN license (pending license accepted).
- Basic Life Support (BLS) certification.
- Ability to provide own transportation for patient visits.
- Familiarity with EPIC/Oasis charting systems.
- COVID vaccination series completed, including one booster.
Preferred Experience / Skills
- Experience with home health patient visits and care planning.
- Comfortable working independently with minimal supervision.
- Good organizational and time management skills.
