Job Title
• Therapist - Social Worker

Schedule / Job Type
• Full-time, Contract, Days, 5 shifts per week

Key Responsibilities
• Provide hospital-based social work services, including discharge planning and care management
• Collaborate with clinical teams across multiple units, including acute care and trauma centers
• Assist patients and families with care coordination and resource planning
• Support patient transitions and ensure compliance with healthcare protocols
• Participate in weekend shifts, providing consistent coverage and support

Required Qualifications
• Valid licensure as a Licensed Clinical Social Worker (LCSW)
• Experience in hospital care management and discharge planning
• Strong communication and interpersonal skills
• Ability to work in a fast-paced healthcare environment
• Knowledge of healthcare documentation systems, preferably Epic

Preferred Experience / Skills
• Experience working in short-term acute care or trauma centers
• Familiarity with hospital operations and patient-centered care processes
• Able to adapt to floating between different hospital units
