Job Title

Key Responsibilities
• Admit patients to hospice services and conduct comprehensive assessments of their physical, psychosocial, environmental, safety, and developmental needs.
• Develop and implement individualized care plans based on patient assessments.
• Document observations, interventions, and patient environment details accurately.
• Coordinate care with a multidisciplinary team to ensure comprehensive patient support.
• Participate in quality assessment and performance improvement initiatives within the hospice program.
• Conduct follow-up visits as needed to monitor patient progress and adjust care plans accordingly.

Required Qualifications
• Valid and active nursing license with eligibility to practice in California.
• Minimum of 1 year of hospice nursing experience.
• Strong assessment skills and ability to develop personalized care plans.
• Excellent communication and interdisciplinary collaboration skills.
• Knowledge of hospice care standards and practices.

Preferred Experience / Skills
• Experience working with patients in the Silicon Valley region (e.g., Sunnyvale, Mountain View, Palo Alto).
• Familiarity with hospice regulations and compliance requirements.
