Job Title
• Therapist - Occupational Therapist

• Santa Rosa, CA
Schedule / Job Type
• Full-time, Days shifts, 8-hour shifts, 5 days per week
Key Responsibilities
• Provide occupational therapy services to residents in a skilled nursing facility and assisted living/independent living environments
• Develop and implement individualized treatment plans to improve residents’ functional abilities
• Collaborate with interdisciplinary teams to ensure optimal patient care
• Document patient progress and maintain compliant records
• Support residents' independence and quality of life through therapeutic interventions
Required Qualifications
• Valid occupational therapist license for the state of California
• Certification in CPR/BLS (Basic Life Support)
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
Preferred Experience / Skills
• Previous experience in SNF, AL, or IL settings
• Knowledge of geriatrics and chronic conditions in older adults
