Key Responsibilities
- Effectively complete and maintain OASIS documentation in accordance with agency policies, regulations, and timely standards.
- Assess patient and caregiver needs utilizing the nursing process to deliver high-quality, evidence-based care aligned with state and federal guidelines.
- Develop and communicate personalized care plans, including patient and caregiver participation, ensuring comprehensive and realistic goals.
- Serve as Case Manager, actively managing caseloads and visit schedules to meet agency productivity requirements.
- Implement medical care plans, nursing interventions, and procedures to maintain patient safety and care quality respecting all regulations.
- Maintain proficiency in high-tech skills such as IV therapy, wound VAC, and complex wound care, ensuring infection control practices are followed.
- Collaborate with LPNs and other healthcare team members to ensure coordinated care, delegate tasks appropriately, and review documentation regularly.
- Evaluate care outcomes, patient responses, and overall care quality, adjusting plans as needed to meet patient needs.
- Ensure accurate and comprehensive documentation of nursing services to support continuity of care and compliance.
- Follow protocols for medication administration, treatment documentation, and visit management using agency systems.
- Educate patients and caregivers on medications, treatment options, discharge instructions, and reportable conditions to facilitate informed decisions.
- Identify and document interdisciplinary needs, communicating effectively with physical therapists, social workers, and other providers.
- Complete all required forms and documentation promptly, including Medicare notices and agency-specific reports.
- Conduct visits efficiently, coordinate with schedulers for timely appointments, and ensure a smooth transition during patient discharges or transfers.
- Assess the need for Home Care Aide services, develop care plans, and supervise ongoing care, ensuring compliance with all regulations.
- Report patient incidents, infections, or injuries promptly to Quality Improvement personnel.
- Coordinate discharges and transfers, ensuring all paperwork and care information are complete and compliant.
- Collaborate with physicians, providers, and team members to deliver comprehensive, coordinated patient care.
- Participate in peer review audits, providing constructive feedback and engaging in continuous quality improvement.
- Organize and facilitate case conferences to promote team communication, documenting discussions accurately.
- Advocate for patient-centered care, ensuring all interventions are tailored and sensitive to individual families’ needs.
Required Qualifications
- Valid Registered Nurse (RN) license.
- Proven experience in hospice or home health nursing preferred.
- Knowledge of OASIS documentation standards.
- Proficiency in high-tech nursing procedures such as IV use, wound vac, and complex wound care.
- Strong assessment, care planning, and documentation skills.
- Ability to collaborate effectively within a multidisciplinary team.
- Excellent communication and patient education skills.
- Knowledge of state and federal healthcare regulations and compliance standards.
Preferred Experience / Skills
- Experience with hospice or palliative care patients.
- Familiarity with McKesson or similar healthcare software systems.
- Experience leading or coordinating case conferences or team meetings.
- Bilingual abilities are a plus.
