Key Responsibilities
- Provide physical therapy services to patients in accordance with physician prescriptions and treatment plans
- Conduct assessments and develop individualized therapy plans
- Monitor patient progress and adjust treatments as needed
- Document patient care and maintain accurate records
- Collaborate with healthcare team to ensure optimal patient outcomes
Required Qualifications
- Valid state license or certification as a Physical Therapist
- Proven experience in outpatient or rehab settings preferred
- Strong communication and patient care skills
- Ability to work independently and as part of a team
Preferred Experience / Skills
- Experience with telehealth services is a plus
- Prior experience in senior care or rehab facilities
- Flexibility to work one weekend rotation per month
