Schedule / Job Type
- Full-time, day shift, with weekend rotation, no call
Key Responsibilities
- Assist in managing the overall care plans for inpatient and outpatient populations.
- Collaborate with patients and their families to ensure comprehensive care coordination across the continuum.
- Participate in assigned shift schedules, either 3-12's or 4-10's, based on team needs.
- Support hospital staff in delivering high-quality patient care and ensuring positive outcomes.
Required Qualifications
- Graduate of an accredited or state-approved Registered Nursing program
- Current licensure as a Registered Nurse (RN) in the state of practice
- Minimum of 3 years of RN case management experience
- Must have received the flu vaccine (no exemptions)
Preferred Experience / Skills
- Prior experience in hospital-based case management or care coordination
- Strong communication and organizational skills
- Ability to work independently and as part of a multidisciplinary team
