Key Responsibilities
- Provide case management and social work services to patients in an acute care hospital setting.
- Collaborate with healthcare teams to develop and implement patient care plans.
- Document patient interactions and treatment plans accurately in electronic health records.
- Support patients and their families through emotional and psychosocial challenges.
- Ensure compliance with hospital policies and healthcare regulations.
Required Qualifications
- Minimum of 2 years of acute care social work experience.
- Must have current clinical license or certification as a Social Worker.
- Experience with Epic electronic health record system.
- Ability to work flexible shifts, including weekends.
- Strong communication and interpersonal skills.
Preferred Experience / Skills
- Prior experience working in a MAGNET-designated hospital.
- Familiarity with hospital charting standards and protocols.
- Ability to work effectively in a fast-paced, team-oriented environment.
