Schedule / Job Type
• Nights, 12-hour shifts, part-time (3 shifts per week)
Key Responsibilities
• Provide bedside care to residents with common diagnoses such as hypertension and diabetes
• Assist with activities of daily living (ADLs) and transfers
• Administer medications and monitor patient conditions
• Document care in electronic medical records (PCC)
• Follow facility protocols and safety procedures
• Collaborate with nursing staff and healthcare team
Required Qualifications
• Valid LVN / LPN license
• Ability to work 12-hour night shifts
• Experience with LTC/SNF patient populations preferred
• Good communication and documentation skills
Preferred Experience / Skills
• Familiarity with PCC electronic medical records
• Experience caring for patients with chronic conditions such as hypertension and diabetes
• Ability to assist with ADLs and medication administration
