Key Responsibilities

• Assess and interview patients, families, and designated individuals within 48 hours of admission to determine comprehensive care needs.
• Develop, coordinate, and monitor discharge plans in collaboration with the interdisciplinary team, patients, and families.
• Communicate effectively with physicians and healthcare providers regarding medical necessity documentation and criteria.
• Educate patients, families, and care teams about post-acute care options, status updates, and care coordination services.
• Arrange and oversee discharge needs and services to ensure timely transitions and prevent delays.
• Participate in daily multidisciplinary rounds to facilitate care progression and target length of stay.
• Monitor and document assessments, interventions, and discharge planning activities in compliance with departmental policies.
• Provide inpatient and outpatient social work services, including psychosocial assessments, care planning, and community follow-up.
• Assist during on-call shifts, weekends, and holidays as needed, including floating within the facility and to locations within 20 miles.

Required Qualifications

• Current nursing license in the State of New Mexico or a Compact State.
• BLS certification from the American Heart Association, American Red Cross, or equivalent.
• At least two years of recent experience as a Registered Nurse in a relevant specialty.
• Successful completion of at least one 13-week travel assignment.
• Ability to communicate effectively and adapt to a fast-paced environment.

Preferred Experience / Skills

• Experience in care coordination, discharge planning, or social work preferred.
• Familiarity with inpatient and outpatient case management practices.
