• Full-time, Day shifts, 5 days per week (Monday through Friday)
Key Responsibilities
• Provide outpatient physical therapy services to geriatric patients in various settings including centers and patients' homes.
• Conduct initial assessments, progress evaluations, and assessments for concerns related to participant care levels.
• Develop and implement individualized treatment plans, collaborating with interdisciplinary teams, families, and care facilities.
• Perform assessments, order DME, and ensure safe mobility and functional independence for aging patients.
• Participate in care coordination meetings, including morning briefings and rehab/clinical conferences.
• Work independently while maintaining close communication with the multidisciplinary team to prevent gaps in care.
Required Qualifications
• Active licensed Physical Therapist (PT) license in Washington state.
• Minimum of 1 year of outpatient therapy experience.
• CPR and Basic Life Support (BLS) certifications.
• Proficiency with Epic electronic health record system.
• Ability to work independently and collaborate effectively with diverse care teams.
Preferred Experience / Skills
• Experience working with geriatric populations in outpatient, home health, or long-term care settings.
• Excellent communication skills and ability to assess and adapt to various care environments.
