Key Responsibilities
• Provide comprehensive hospice care to patients in a home setting.
• Manage hospice cases, including assessment, planning, and coordination of care.
• Document patient interactions and care plans accurately using electronic medical records.
• Collaborate with interdisciplinary teams to ensure quality of care.
• Travel to patient homes, including rural areas, for scheduled visits.
• Follow all hospice protocols and maintain compliance with clinical standards.
Required Qualifications
• Valid RN license in the state of Vermont.
• Experience with hospice case management essential.
• Prior home hospice care experience required.
• Ability to travel to rural areas as needed.
• Knowledge of EMR systems, preferably McKesson or Netsmart.
• Capable of working a schedule from Sunday to Thursday.
Preferred Experience / Skills
• Experience working in rural travel environments.
• Familiarity with EMR platforms like McKesson or Netsmart.
