LVN / LPN LTC/SNF
Job Title
- LVN / LPN - Long-Term Care / Skilled Nursing Facility

- Berlin, VT
Schedule / Job Type
- Full-time, 12-hour night shifts, 3 shifts per week, temporary assignment (16-32 weeks)
Key Responsibilities
- Provide nursing care to residents in a long-term care or skilled nursing setting.
- Float to different units as needed, including dementia care populations.
- Maintain compliance with facility protocols and patient care standards.
- Work at least every other weekend and participate in holiday coverage, including Thanksgiving, Christmas, Christmas Eve, and New Year’s Day.
- Collaborate with multidisciplinary team to ensure quality patient care.
Required Qualifications
- Valid LVN or LPN license.
- A minimum of 3 years of nursing experience.
- BLS certification through AHA.
- Prior travel nursing experience preferred.
- Experience working in LTC facilities required.
- Willingness to float between units as needed.
- Comfortable working with dementia patients.
Preferred Experience / Skills
- Experience working in a skilled nursing or long-term care environment.
- Familiarity with electronic medical records (EMR).
- Ability to work flexible schedules with no block scheduling.

- Travel compensation rates vary based on miles and local positioning, with specific rates for travel, radius, and local assignments.
- All work history details, including bed count, EMR, and facility type, must be provided to proceed.
- Reliable transportation is required, as public transit is not accessible.
- Vehicles should be equipped for winter conditions from November to April.
- All employment gaps over 90 days must be explained.
- Bi-weekly orientations are scheduled, with the next session on 12/08.
- All assigned hours must be worked; preplanned time off requests are to be communicated upfront.
