Key Responsibilities
- Assess patient and caregiver needs utilizing the nursing process to deliver high-quality, evidence-based care that complies with all relevant regulations and policies.
- Develop and communicate personalized care plans, ensuring patient and caregiver involvement in the process.
- Serve as the primary case manager, actively managing caseloads and visit schedules in accordance with agency productivity standards.
- Complete accurate and timely documentation, including OASIS assessments, fully adhering to agency, state, and federal guidelines.
- Implement medical plans of care and nursing interventions, ensuring safe, effective, and quality patient care.
- Stay current on high-tech skills such as IV therapy, wound vac, and complex wound care, maintaining infection control standards throughout care delivery.
- Collaborate with Licensed Practical Nurses (LPNs) to follow care plans, delegate tasks appropriately, and ensure patient needs are consistently met.
- Evaluate the effectiveness of care plans, monitor patient responses, and adjust interventions accordingly to meet individual needs and maintain quality standards.
- Educate patients and caregivers on treatment options, medication side effects, discharge instructions, and reportable conditions, enabling informed decision-making.
- Coordinate holistic care by identifying interdisciplinary needs, documenting patient requirements, and communicating efficiently with relevant team members.
- Complete necessary forms and reports required by agency, state, and federal agencies, ensuring accuracy and timeliness.
- Conduct patient visits efficiently, coordinating with supervisors to maintain quality standards and manage schedules effectively.
- Assess patients’ needs for home care services, develop care plans collaboratively, and provide ongoing supervision in line with regulations.
- Report incidents such as infections or injuries to the Quality Improvement (QI) team promptly.
- Manage patient discharges or transfers, ensuring all documentation and care transitions adhere to policies.
- Collaborate with physicians, healthcare team members, vendors, and other providers to support comprehensive, safe, and effective patient care.
- Participate in monthly peer review audits, providing and receiving constructive feedback professionally.
- Lead or participate in case conferences to facilitate interdisciplinary communication and optimal patient outcomes.
- Advocate for patients to ensure that care provided respects individual needs and preferences.
- Conduct comprehensive assessments of patient and caregiver needs upon admission and regularly thereafter, updating care plans as needed within established timeframes.
- Maintain state-of-the-art knowledge of pain management, symptom control, IV therapy, wound care, and infection control practices.
- Educate patients and caregivers to ensure understanding of treatment options, discharge plans, and reportable conditions.
- Coordinate multidisciplinary care efforts involving physicians, providers, and team members to ensure safe and seamless care delivery.
Required Qualifications
- Valid Registered Nurse (RN) license in the state of Vermont
- Knowledge of hospice and home healthcare standards and regulations
- Strong assessment and documentation skills, including experience with OASIS and other federal forms
- Ability to perform high-tech skills such as IV therapy, wound care, and complex wound management
- Excellent communication and patient education skills
- Ability to work independently and as part of an interdisciplinary team
- Valid driver’s license and reliable transportation
Preferred Experience / Skills
- Previous hospice or home healthcare experience
- Experience with high-tech nursing equipment such as wound vacs and IV pumps
- Bilingual abilities (preferred, but not required)
- Experience participating in interdisciplinary team meetings and care coordination
