Key Responsibilities
- Provide outpatient physical therapy services to home health patients.
- Evaluate patient needs and develop individualized treatment plans.
- Assist patients in improving mobility, strength, and overall function.
- Document patient progress using electronic charting systems (EPIC, Rover, Oasis).
- Collaborate with interdisciplinary team to ensure quality patient care.
- Maintain up-to-date documentation and compliance with regulations.
Required Qualifications
- Valid state license as a Physical Therapist (pending license acceptance acceptable).
- At least 1 year of skilled home health experience.
- BCLS certification.
- Own transportation to visit multiple patients across the assigned radius.
Preferred Experience / Skills
- Experience with Oasis documentation.
- Comfortable working in a home health setting and managing travel within a 60-mile radius.
- Familiarity with EPIC and Rover systems.
