Key Responsibilities
- Collaborate with a multidisciplinary team to support inpatient Care Coordination and Utilization Management.
- Lead local inpatient interdisciplinary team meetings within the Critical Access Hospital (CAH) Swing Bed/Transitional Care program.
- Review admissions to develop personalized, clinically appropriate treatment plans aligned with patient needs, preferences, and resource availability.
- Coordinate with providers, leadership, and support staff to ensure treatment plans meet regulatory and payor criteria.
- Conduct patient assessments, provide psycho-social support, and facilitate referrals as needed.
- Develop and communicate discharge plans, ensuring safe and coordinated transitions of care.
- Support peer review processes in accordance with consortium guidelines.
Required Qualifications
- Current full, unrestricted Registered Nurse license in Alaska or another U.S. state; applicants must have applied for an Alaska license before their start date.
- Bachelor’s degree in Nursing preferred.
- Case Management Certification within 24 months of hire from a recognized certifying body.
- BLS certification; ACLS and PALS preferred.
- Clinical competency certification within 3 months of hire and every three years.
- High school diploma or equivalent required.
Preferred Experience / Skills
- Minimum of 5 years of clinical nursing experience.
- At least 1 year of experience in chart review, risk management, or related quality services.
- Knowledge of medical record review, insurance guidelines, and regulatory compliance.
- Strong communication and organizational skills.
- Ability to work collaboratively as part of an interdisciplinary team and build relationships with colleagues, community partners, and patients.
Physical Demands
- Regularly required to talk, hear, stand, walk, sit, and use hands.
- Occasionally required to climb, balance, stoop, kneel, crouch, or crawl.
- Must be able to lift and move up to 50 lbs.

- Case Management Certification (Commission for Case Manager Certification)
- Clinical Competency and High Risk Competency (SEARHC)
