Key Responsibilities
- Manage the active care coordination and case management for patients, collaborating with healthcare providers and interdisciplinary teams to establish personalized medical goals.
- Facilitate and monitor patient referrals to specialty clinics or surgical services, ensuring all relevant documentation is complete and tracking referral progress and outcomes.
- Serve as the primary clinical contact for patient care, proactively addressing delays and obstacles to ensure timely and effective treatment.
- Perform clinical nursing duties including patient assessment, documentation, education, and administration of medications or therapies, while maintaining compliance with safety and infection control standards.
- Maintain annual competencies in safety and infection control, and ensure a safe, clean working environment with proper supplies and functional equipment.
- Communicate effectively with team members, participate in departmental and organizational improvement initiatives, and support a collaborative team environment.
Required Qualifications
- Current unrestricted Registered Nurse license in Alaska or another U.S. state (must have applied for Alaska license if not yet licensed).
- Graduate of an accredited nursing school.
- Basic Life Support (BLS) certification from the American Heart Association.
- Clinical Competency certification within 3 months of hire and every three years.
- Knowledge of nursing processes, patient safety, infection control principles, and strong assessment skills.
- Excellent oral and written communication skills, with the ability to multi-task and work independently in a fast-paced environment.
- Ability to lift and move up to 50 lbs.
Preferred Experience / Skills
- Bachelor’s degree in nursing.
- Experience in care coordination, patient referral management, or outpatient clinic settings.
- Strong problem-solving and critical thinking skills, with a team-oriented mindset.
- Willingness to teach and support interdisciplinary collaboration.
