- Full-time, 12-hour shifts, 3 shifts per week (days)
Key Responsibilities
- Provide patient care to a long-term care and skilled nursing facility population.
- Assist with patient assessments, medication administration, and documentation.
- Collaborate with healthcare team to ensure high standards of care.
- Follow facility protocols and maintain compliance with regulatory standards.
- Support patient comfort and safety during assigned shifts.
Required Qualifications
- Valid LVN or LPN license in the state of Minnesota.
- Experience in long-term care or skilled nursing facilities preferred.
- Ability to work 12-hour night shifts.
- Strong communication and teamwork skills.
- Proficiency with electronic medical records (EMR), specifically PCC.
Preferred Experience / Skills
- Prior experience with general long-term and rehab patient populations.
- Ability to adapt to varying patient ratios and care needs.
- Familiarity with respiratory or rehabilitation care is a plus.
