Registered Nurse - LTC/SNF
Location: Canby, MN
Schedule / Job Type: Full-time, Midshift, 8-hour shifts, Monday through Friday (5 days/week)
Key Responsibilities:

• Provide nursing care to residents in a long-term care or skilled nursing facility setting.
• Assess, monitor, and document patient conditions, ensuring quality care delivery.
• Administer medications and treatments following facility guidelines.
• Collaborate with interdisciplinary teams to develop and update care plans.
• Ensure compliance with all regulatory and safety standards.
• Maintain accurate EMR documentation using Matrix or paper flowsheets.
  Required Qualifications:
• Active Minnesota RN license.
• Registered nurse experience in LTC or SNF settings preferred.
• Ability to work primarily 8-hour shifts, including every other weekend and holiday.
• Strong assessment and documentation skills.
• Must have active MN SNSA registration.
  Preferred Experience / Skills:
• Experience caring for dementia patients.
• Ability to adapt to facility policies regarding patient ratios and care flow.
• Comfortable working in a two-story facility with varied patient acuity levels.
