Job Title

Schedule / Job Type
- Travel, 13-week assignment, full-time, 12-hour day shifts (7:00 AM – 7:00 PM)
Key Responsibilities
- Provide direct patient care, including bathing, feeding, toileting, and ambulation for elderly and behavioral health patients
- Deliver person-centered care, especially for dementia or behavioral health patients
- Assist with end-of-life care according to care plans
- Collaborate with healthcare team members to ensure quality patient care
- Maintain accurate documentation using CareTracker LTC EMR
Required Qualifications
- Valid CNA certification is required
- Minimum of 2 years of experience as an LVN/LPN in a medical-surgical or long-term care setting
- Certification as Medication Aide preferred
- CNA II certification a plus
Preferred Experience / Skills
- Experience with long-term care facilities
- Experience assisting patients with dementia or behavioral health needs
- Ability to adapt to fast-paced environments and elderly patient care
