Key Responsibilities

  • Provide hospice and end-of-life care to adult and pediatric patients in home and inpatient settings.
  • Assess and manage complex symptoms, utilizing critical thinking skills for symptom assessment and care planning.
  • Participate in goals of care discussions with patients, families, and the healthcare team.
  • Administer medications via IV, subcutaneous, oral, sublingual, and rectal routes, ensuring safe medication management.
  • Document patient care accurately in an EMR system, with heavy emphasis on narrative entries.
  • Cover on-call duties occasionally, with flexibility to work 2 out of 4 major holidays.

Required Qualifications

  • Valid RN license.
  • AHA BLS certification.
  • Minimum of 3 years of clinical experience in community hospice or palliative care.
  • Strong documentation skills and proficiency with electronic medical records (EMR).
  • Ability to work independently in a home health/hospice setting.

Preferred Experience / Skills

  • Prior home health or hospice travel experience.
  • Experience with complex symptom management, ventilator support, and use of specialized equipment (e.g., IV pumps, BiPAP, CPAP).
  • Ability to handle high-acuity patients and participate in multidisciplinary team planning.

Additional Notes

  • Work in a facility with a census of approximately 100 patients, including inpatient and outpatient hospice care.
  • Reliable transportation is essential; personal or rental vehicle strongly recommended, especially during winter months with snow tires.
  • Must have excellent typing skills for EMR documentation and be comfortable working with end-of-life protocols and equipment.
