Job Title

  • Full-time, night shifts, 3 shifts per week

Key Responsibilities

  • Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting
  • Float to different units as needed, including dementia care units
  • Collaborate with healthcare team to implement patient care plans
  • Document care and observations accurately in electronic medical records
  • Ensure compliance with all safety and healthcare regulations
  • Work every other weekend as part of the schedule

Required Qualifications

  • Valid LVN or LPN license
  • Minimum of 3 years of experience in long-term care (LTC) environments
  • Current BLS certification through AHA
  • Prior travel experience preferred
  • Must be comfortable working with dementia patients
  • Ability to float between units as required

Preferred Experience / Skills

  • Experience working in skilled nursing facilities or similar settings
  • Familiarity with electronic medical records systems
  • Valid background checks including nationwide, county court searches, FACIS3, and sex offender registry within the last 18 months
  • Ability to explain employment gaps over 90 days

Additional Information

  • Travel rate: $75; Radius: $70 (51-100 miles); Local: $65 (50 miles or less), measured in a straight line
  • No block schedules; all shifts are 12 hours
  • Reliable transportation is required; personal or rental vehicles recommended, equipped for winter conditions from November to April
  • All work history details (facility size, EMR systems, etc.) must be complete to proceed with submission
  • All employment gaps over 90 days must be explained upfront
  • This facility requires all documentation for background checks and compliance before onboarding
  • Please be aware of the importance of working scheduled hours and communicate any preplanned time off prior to start
