Registered Nurse - LTC/SNF
Location: Mauston, WI
Schedule / Job Type: Full-time, Days shift, Onsite, 8-hour shifts, 5 days per week

Key Responsibilities:

• Provide compassionate nursing care to residents in a long-term care or skilled nursing facility setting.
• Communicate promptly with physicians, families, and team members regarding resident condition changes.
• Complete all resident assessments and documentation accurately and thoroughly.
• Administer medications following the 5 Rights of medication safety.
• Perform wound care and assessments as needed.
• Assist CNAs with call light response and monitor residents for changes in health status.
• Act as a floor leader by assessing and managing situations to ensure resident safety and quality care.
• Report any allegations of abuse, neglect, or misappropriation to supervisory staff.
• Maintain detailed and accurate resident records and charting.
• Perform additional duties as assigned.

Required Qualifications:

• Two-year nursing degree (Associates or equivalent) required; Bachelor’s degree preferred.
• Valid Wisconsin Registered Nursing license and current CPR/BLS certification.
• At least 1 year of relevant nursing experience preferred.
• Exceptional attention to detail and accuracy.
• Navy Blue scrub attire.

Preferred Experience / Skills:

• Experience in long-term care or skilled nursing facilities.
• Strong communication skills and ability to assess and respond to changing resident needs.
