Therapist - Physical Therapist
Location: Elkins, WV
Schedule / Job Type: Full-time, 5 days per week, days shift (8:30 AM - 5:00 PM), no weekends or call, 13-week contract
Key Responsibilities:

• Provide outpatient physical therapy services to home health patients in a designated service area
• Conduct assessments, develop and modify treatment plans, and evaluate patient progress
• Document care plans, assessments, discharge summaries, and reassessments in compliance with regulation standards including OASIS and RUG levels
• Educate patients and their families on therapeutic exercises and injury prevention strategies
• Manage therapy equipment and assistive devices assessments such as gait belts, hospital beds, and wheelchairs
• Coordinate care in accordance with physician orders and regulatory requirements, including timely reassessments and long-term goal adjustments

Required Qualifications:

• Valid WV State Physical Therapist License
• BLS certification
• Minimum of 1 year of relevant home health experience
• Knowledge of OASIS and EHR systems such as Netsmart (McKesson)

Preferred Experience / Skills:

• Experience with orthopedics, neurologic rehab, and post-stroke recovery
• Skills in gait training, assistive device fitting, and environmental assessments
• Ability to educate patients and families on home exercise programs and safety measures
