Key Responsibilities
- Provide occupational therapy services to patients in a Skilled Nursing Facility (SNF) setting.
- Assess patient needs and develop personalized treatment plans.
- Implement therapeutic interventions to improve patient functional abilities.
- Collaborate with healthcare teams to ensure coordinated patient care.
- Document patient progress and maintain accurate medical records.
Required Qualifications
- Minimum of 1 year of professional SNF experience.
- Valid occupational therapy license in the state of practice.
- Strong knowledge of occupational therapy practices and procedures.
Preferred Experience / Skills
- Experience working with a diverse patient population.
- Ability to work independently and as part of a multidisciplinary team.
