• Days, part-time, 4 shifts per week, 10 hours per shift

Key Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs.
• Restore function to prevent or minimize disability following disease or injury.
• Assist clients in reaching their maximum performance within their capabilities.
• Supervise Physical Therapy Assistants and perform initial admission/evaluation visits.
• Document observations, clinical findings, treatments, goals, and discharge plans.
• Notify the Clinical Team Manager of OASIS scores within established timelines.

Required Qualifications

• Minimum of one year of experience in home health physical therapy.
• Valid license to practice as a Physical Therapist in California.
• Knowledge of medical and therapeutic procedures related to physical therapy.
• Ability to evaluate and develop appropriate care plans for patients.

Preferred Experience / Skills

• Experience working in home health or outpatient settings.
• Strong documentation and communication skills.
