Key Responsibilities
- Assess, plan, and implement nursing care for residents under supervision of an RN.
- Provide direct patient care including passing medications and monitoring residents’ health status.
- Supervise Certified Nursing Assistants (CNAs) and support staff to ensure quality care.
- Ensure continuity of care through effective communication and coordination.
- Maintain documentation in Electronic Medical Records (EMR) accurately and timely.
Required Qualifications
- Valid California LVN / LPN license.
- Prior experience in long-term care (LTC) or skilled nursing facility (SNF).
- Proficiency in Electronic Medical Records systems.
- Current BLS certification.
- Ability to pass nursing medical tests.
- Excellent verbal and written communication skills in English.
- Good interpersonal skills and professional conduct with patients, staff, and external agencies.
- Basic computer skills.
Preferred Experience / Skills
- Previous experience supervising staff in a healthcare setting.
- Familiarity with state and federal long-term care regulations.
