Key Responsibilities
- Provide dementia-capable nursing care within a memory care unit
- Administer medications and treatments following care plans
- Collaborate with interdisciplinary team to ensure quality resident care
- Document patient status and interventions accurately in electronic health records
- Support residents with activities of daily living and behavioral needs
- Adhere to facility policies regarding infection control and safety protocols
Required Qualifications
- Valid LVN or LPN license
- Minimum of 1 year clinical experience in long-term care or skilled nursing facility
- Prior travel nursing experience preferred
- Experience using EPIC electronic health record system within the past 2 years
- Ability to work 8-hour mids shifts, including the ability to work all holidays
Preferred Experience / Skills
- Dementia capable care certification or training
- Experience with phlebotomy, EKGs, and transport procedures
- Reliable transportation, personal or rental vehicle with all-season or snow tires for winter months

Additional Notes
- No guaranteed weekends or days off unless specified in contract; floating shifts required
- Must be able to work 2 out of 4 major holidays (Thanksgiving, Christmas, Christmas Eve, New Year’s Day)

- The facility does not have accessible public transportation; reliable transportation is essential
