LVN / LPN LTC/SNF

Schedule / Job Type
- Full-time, 8-hour shifts, Mids, 4 shifts per week
Key Responsibilities
- Provide dementia-capable, compassionate care for memory care residents within a Long-Term Care/SNF environment
- Administer medications and perform assessments using EPIC electronic medical records system
- Conduct routine procedures such as phlebotomy, EKGs, and patient transports
- Collaborate with clinical team to ensure quality resident care and safety
- Support residents with Alzheimer’s and other memory impairments, following specialized protocols
- Work flexible schedules, including working 2 out of 4 major holidays
Required Qualifications
- Valid LVN/LPN license in the state of Vermont
- Minimum of 1 year of clinical experience, preferably within a memory care or LTC setting
- Prior travel experience required, with recent EPIC EMR experience (within past 2 years)
- Successful completion of a 7-year background check (nationwide, county court searches, FACIS3, sex offender registry) within 18 months of start date
- Ability to work all holidays as required
- Reliable transportation, personal or rental vehicle recommended, equipped with all-season or snow tires during winter months
Preferred Experience / Skills
- Dementia capable care certification
- Experience with EPIC EMR system within the past 2 years
- Knowledge of Long-Term Care unit protocols and procedures
