Therapist - Social Worker

Location: Albany, OR

Schedule / Job Type: Full-time, direct hire, onsite, 8-hour shifts, 5 days per week

Key Responsibilities:

• Provide social work services to hospice patients and their families in various settings, including private residences and care facilities
• Support patients through emotional, spiritual, and psychosocial assessments and interventions
• Collaborate with medical teams to develop and implement comprehensive care plans
• Assist with grief counseling and bereavement support for families
• Maintain accurate documentation of services provided in compliance with healthcare regulations
• Utilize personal vehicle for travel to patient locations as needed

Required Qualifications:

• Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) license
• Healthcare Provider Level BLS certification
• Valid driver's license with auto insurance and good vehicle maintenance

Preferred Experience / Skills:

• LCSW (Licensed Clinical Social Worker) licensure in Oregon
• Experience working in hospital or medical settings with social work responsibilities
